The Upright fall prevention program is designed to license your healthcare organization, practice settings, providers, and patient populations, with a fixed annual rate, during the term of the agreement.  The licensure agreement includes:

• unlimited access to CE–approved, online education designed for all care providers;
• partner support and additional fall prevention resources; and
• pre-built pathways to integrate the Hendrich II Fall Risk Model into your existing EHR platform or Assessment Form.

Online education

Unlimited use of CE-approved, online education designed for licensed and non-licensed care providers. The program is didactic, interactive, and tests critical thinking in various scenarios to elicit fall prevention tactics and risk assessments.

Integrate the program

Design and build the Hendrich II Fall Risk Model into your existing EHR platform or Assessment Form to assess and document patient risk, and align interventions to reduce fall risk, based on person-centered care development plans. The Upright fall prevention program recommends interdisciplinary care and high reliability practices with a team-based approach that is person-centered care and continuum-focused.

Identify risk factors

By utilizing the Upright fall prevention program, your care provider will efficiently learn the following:

• how to objectively assess mobility and gait;
• classes of drugs that contribute to fall risk;
• the importance of elimination management; and
• the relationship of cognition and fall risk.

Investigate & analyze

Continuous monitoring of each fall event provides insight into failure modes that result in the true cause of remaining falls. Fall huddles can provide valuable information at the care level to drive elimination of injurious fall events. Utilize fall huddles or debriefs with control charts to reflect on fall incidents and incorporate the findings into your program.

Engage care providers in your organization

Care providers have a unique opportunity to take a proactive approach to reduce injurious fall events. Care providers should anticipate that a fall event is likely to occur based upon identified risk factors that are present with the individual. Care providers can than apply the appropriate interventions to prevent the fall from occurring in the first place.
